Acrobat Reader is required to properly fill-out and submit the form to us by email. If you choose to submit the data to us via email, please be advised of the following:

1. OPTION 1 - Uses an email client on your computer (IF CONFIGURED) to send an XML "attachment". Leave all information as-is and send the email.
   
2. OPTION 2 & 3 - Both require you to SAVE the data file to your computer THEN send it manually to us using another email client (i.e. Yahoo, MSN, AOL, Hotmail, etc...). You must attach the XML file yourself if this option is chosen.
